Required Tournament Documentation

1)               State or National Association Certified Team Roster (2 copies)

2)               Tournament Roster and Release Form – from tournament website at Forms link

3)               State or National Association issued Player Pass for each team member with photo ID

4)               Coach Pass for each coach or manager who will be on sideline with team

5)               Individual Medical Release for each player

6)               USYSA Permission to Travel or Intent to Travel Form (for non-S.CY.S.A. teams, if required by your state association)

7)               Emergency Contact and Visitor Questionnaire Form – from tournament website at Forms link
